Pigmentation means excessive deposition of the natural skin pigment in some areas. It can be caused by UV light Exposure, Hormonal Imbalance, or reactivation of Pigment cells after any injury or rash called as Post-Inflamatory Pigmentation (PIH).
Pigmentation can be effectively treated with various modalities like peels, lasers and creams depending on the type and depth of Pigment.

Common Pigment conditions are Freckles or Sunspots:

Freckles are small dark spots which appear on cheeks and nose. It is genetic, usually runs in families. They appear with age and with cumulative exposure to sunlight. They may appear in young age if you have strong genetic tendency. Regular use of sunscreen to protect your skin from UV  damage and Vitamin C serum (as it is a strong antioxidant prevents oxidative damage and prevents further freckling to some extent.

What are the Treatments available for freckles?

Regular topical De-Pigmenting Creams with kojic acid, arbutin or azelic acid may help to lighten them to some extent in early stage. As freckles are in deeper layers of skin, they may not reduce with topical creams.

Laser is the main stay of treatment to remove freckles. QS Ndyag Laser, 532 KTP laser and 560 Nm Lasers are usually used sucessfully to clear the freckles. Requires single session and a second session after a couple of months if any required.

Melasma is the blotchy pigmentation on face, usually on cheeks and nose. Sometimes can be on forehead and chin also. It is usually because of hormones, normally it starts after 35 years, Pre-Menaupausal, During Pregnancy and also to women using birth control pills. It will be aggravated by Sun Exposure.

What are types of Melasma ?

Epidermal Melasma - Pigment will be superficial in the upper layers of the skin (Epidermis)

Dermal Melasma - Pigment in the deeper layers of the skin (Dermis)

Mixed Melasma - Pigment will be both epidermal and dermal.

What are the treatment options available for Melasma ?

If the Melasma pigmentation is superficial in the epidermal layers, topical creams will improve the pigmentation. If the pigment is in the dermis (the deeper layers of the skin) then creams may not help. Chemical peels of medium depth can help clearing the spot. QS Nd Yag laser therapy is the treatment of choice for recurrent Melasma. It may require 8-10 sessions. Erbium Yag laser can also help in reducing the Pigmentation.

Is Melasma Curable?

As Melasma is Hormone dependant when treated it reduces and may re-appear again. Once cleared Melasma can be kept under control using maintenance creams if it is superficial or maintenance treatment sessions are required once in couple of months if it is the deep dermal type to prevent recurrence. Pregnancy pigmentation will be cleared after the child birth.

How can I prevent dark spots after Acne?

Acne is an Inflammatory condition,  so may leave a dark spot while healing. If you have that tendency to heal with spot when you have a minor cut or burn, you can prevent it to some extent by protecting with sunscreen and not by picking pimples which will aggravate inflammation and pigmentation. Also your dermatologist may prescribe some night creams like glycolic, retinoic or azellic acid creams depending on your skin type and severity of your problem which will help reduce breakouts as well as spots.

If the spots and acne are severe and if have an uneven skin tone too, your dermatologist may advice you salicylic, glycolic, peels or laser skin toning to achieve even and flawless skin.
